CHILD AND FAMILY TEAM
The Professional Partner utilizes the wraparound process in order to build a team dedicated to flexible and unconditional care. A Child and Family Team is developed for each child/adolescent and family served in the program. The youth and family identifies those individuals they desire to participate on the team. The Child and Family Team generally consists of individuals who know the family best and are willing to work to help the child and family have a better life.

The family is seen as the expert regarding their children while professional act as consultants to the family.
Decisions are reached by consensus. All members have input into the Individual Family Support Plan (IFSP) and all Team members have ownership of the plan.

ASSESSMENT, INDIVIDUAL FAMILY SUPPORT PLAN DEVELOPMENT, AND MONITORING PROCESS
Team meetings should be scheduled to allow the key Team members to be present. Meetings will not be held without family members and should last an average of 60 minutes. All information submitted to the Team must be treated in a confidential manner. Issues and information concerning the child/family situation must be shared with the participating Team members only. The Team will identify the child and family's strengths, goals, and needs. Life domains to be addressed by the team on the Individual Family Support Plan include:
Life domains are those aspects of life in which almost all humans experience need to a day by day basis. After reviewing the life domains, the Team will select the top priority domains, in addition to the safety domain. Parents have veto rights regarding all selection proceeding. The process of developing the Individual Family Support Plan will then begin.
