Reporting and Data Collection Requirements

The Professional Partner Program will report on an annual basis to the Regional Governing Board, the Department of Health and Human Services, and referring agencies the outcome information described in these guidelines.

For each youth and family served, the Professional Partner Program will report the date of acceptance into and discharge from the Professional Partner Program.

The Evaluation Specialist at Region 3 Behavioral Health Services will train and monitor data collection efforts by Professional Partners. As part of job responsibilities, Professional Partners will be required to collect a number of assessment tools from youth, their families and schools.

1. At intake and 6 month intervals for all youth:  
  Descriptive Information Questionnaire DIQ
  Child Behavior Checklist CBCL
  Child & Adolescent Fuctional Assessment Scale CAFAS
  Functional Impairment Assessment (intake only) FIA
     
2. During services on quarterly basis:  
  Ohio Scales  
     
3. During services on weekly basis:  
  Weekly Adjustment Indicators WAI
     
4. Additional tools that will be utilized:  
  Wraparound Fidelity Instrument WFI
