Discharge Criteria

Discharge from the Professional Partner Program is determined by the Child and Family Team. Discharge occurs when:

  • The youth aged out
  • The youth is deceased
  • The family expressly refused services
  • The family passively refused services
  • Less intensive services are needed
  • The youth moved outside of the state/region
  • The youth is placed out of the home
  • Services are no longer needed
  • The youth becomes a state ward in the Integrated Care Coordination Unit (ICCU)
  • Professional Partner is unable to locate the client
  • Unplanned termination from the program
  • Youth request negative
