INDIVIDUAL FAMILY SUPPORT PLAN

The Child and Family Team creates the Individual Family Support Plan (IFSP) that includes family goals and Safety planning. The Team supports the implementation of the IFSP and inspires unconditional commitment to the child and family. The Team also identifies and implements informal supports that will be important to the success of the family long after formal services are gone. These supports are designed to remain with the family far beyond the time the family receives formal services.

The IFSP shall include:

  • The short-term and long-term goals for treatment and support services for the child and family
  • The roles and responsibilities of individual team members
  • Clear and measurable steps to reach identified goals
  • Monitors for monthly progress
  • Identification of formal and informal resources
  • Revisions are made by all team members on a regular or as needed basis

Guidelines for IFSP Implementation

  • In the process of developing the IFSP, it will be reviewed by all team members. Once approved the plan will be signed and implemented.
  • The professional Partner will advocate for the process ensuring all team members are provided the opportunity to participate in assessment, planning, and ongoing review of services to the fullest extent possible.
  • Support should be community-based in the least restrictive and least restrictive manner.
