Youth Emergency Community Support Services

In an on-going effort to achieve the outcomes of Youth ERCS, the program provides the following services:

  1. Assist the consumer with referral to necessary services and supports
  2. Teach  the consumer daily living skills
  3. Prepare a Strength Based Assessment, Safety Plan and Individual Program Plan with the consumer
  4. Promote communication and collaboration between existing providers and family members
  5. Provide assistance with accessing medications
  6. Initiate/attend treatment team and discharge meetings
  7. Facilitate the completion or progress  steps towards the goals outlined in the Individual Program Plan and Strength Based Assessment  within 90 days
  8. Meet with consumer frequently in their chosen setting
  9. Promote consumer stabilization and recovery
  10. Enhance the quality of life of the persons served
  11. Reduce symptoms or needs and build resilience
  12. Restore and/or improve functioning
  13. Support the integration and transition of the persons served into the community.
  14. Coordination with supportive family/legal guardians.
